Vision benefits are provided through VSP. Collective Health will send you an ID card.
Retailers such as Costco, Visionworks and Cohen's Fashion Optical are included in VSP's retail chain providers, and services from these providers are considered in-network. See a complete list of VSP's retail chain providers at vsp.com.
Lenses, frames and contacts (in lieu of lenses and frames) are covered once per calendar year.
